When a patient is directly referred, the PT can discuss the patient’s case with the referring provider.

To receive services from an OT, an order must be signed by a KY Medicaid participating physician, advanced practitioner registered nurse, physician assistant or psychiatrist.

What do I need to wear or bring with me?
